Pay-As-You-Go Packages Purchase Option

Rather than paying full Package Price (excludes Plans) upfront, you may elect to purchase any Package (Not a Plan) by:

paying an initial Charge of 10% of the total Individual HBOT or PBM Packages price,
sign an agreement to purchase the entire Package,
pay the stated discounted session price listed on our Services Menu Pricing after each session you complete.
The Initial Charge payment is non-refundable and non-transferable.

Refunds

Package or Plan Purchases are non-refundable, but remaining unused prorated balances can be applied to any individual sessions prices if Client is unable to complete a Package or Plan.

Canceling Packages

Citizen OxyWellness reserves the right to cancel any Session, Package, Plan or Membership at any time with a refund to Client of any unused amounts paid for such Session, Package, Plan or Membership on a pro-rata basis.

Health Insurance Reimbursement

At this time we cannot prepare, file or submit any forms for reimbursement of our Services cost to Health Insurance companies or to Medicare or Medicaid. All payments for our Services must be made by You personally at the time of Service. Some of our Services may be eligible for reimbursement if properly submitted for reimbursement by you directly to your insurer. We can provide you with appropriate invoices detailing our Services performed and costs thereof. Most such eligible reimbursement requests would require a physician's prescription and may require FDA approval of the use of our type of Services for a particular approved medical condition in order to be eligible for reimbursement.
